on the immunization form we have to fill out for the forum, there are a lot of shots listed. do we need to have ALL of those immunizations done in order to visit some of the hospitals? or are those simply all the shots that you could have possibly gotten and the forum just wants to know which ones you've had?

for example, do we need to have the polio vaccine? i thought that disease basically died out, so would it be okay if we don't have it? would it affect which hospitals we go see?

With a few exceptions, most people have had all the shots that are listed on that sheet. That polio one is one that kids get as very young kids. If you were to look through your shot record, you more than likely got all of them, unless your parents chose to not have you vaccinated. They're not shots that you must get now; they're shots that are normally given earlier in life
If you haven't gotten the shots, I'd call the office and just find out what that does to you. I really don't know
