You need to send the travel info paper back. It tells them how you're getting there. I *think* but read the paper to check for sure that you bring the medical form with you on the first day of the forum. That should be it for papers that need to be sent/brought, but just glance through and make sure that there are no others.

Yes, I think that is supposed to be the nametag. Check the back, it may have something you peel away to turn it into a sticker

Yes, write your name on it. That's basically just used once you arrive at the airport (don't wear it on the plane). They'll give you a nametag once you check in at the forum that lasts longer
